Required Job Qualifications:

  • Graduate of an approved LPN program.
  • Current and active LPN license through the Minnesota Board of Nursing.
  • CPR Certified.
  • Proven ability to exercise discretion and independent judgment in affecting/directing patient care in an Assisted Living setting.

Preferred Qualifications:

  • 1 year Assisted Living or Long Term care experience
  • Previous experience in providing effective work direction and in a home-like setting.
  • Demonstrated ability in providing high quality clinical care in a customized or Assisted Living setting.
  • Basic understanding of Medicare/Medicaid and third party payer programs.

Job Summary:

  • Provides professional nursing to tenants in our apartments as an hourly, non-exempt professional.  Effectively lead and coordinate unlicensed care staff.  This position requires advanced knowledge of nursing and effective compliant administrative practices.  It requires the consistent exercise of independent judgment and discretion to provide and ensure the delivery of high quality care to tenants through effective management of systems and staff. 

Essential Responsibilities and Functions:

  • Assist tenants in achieving and maintaining highest functional levels.
  • Communicates with Pharmacy regarding prescription medications ordered for tenants.
  • Concise and accurate documentation related to tenant medication distribution. 
  • Lead staff in effective delivery of services and communications to tenants afflicted with Alzheimer’s disease and memory loss.
  • Proper administration of medication (without error).
  • Report any changes in tenant condition to supervising Registered Nurse.
  • Review and maintain tenant care plans.
  • Meet with social workers/county workers as needed for services and to update any in tenant change in status.
  • Demonstrates effective communication with residents, families, staff, physicians, supervisors, and other departments.
  • Working on the floor due to open shifts or call-ins, as needed.
  • Demonstrates teamwork through attitude, assistance, and cooperative approach to working with residents, visitors, staff, and supervisors.
  • Satisfactory performance of other tasks assigned by supervisor.
  • Must also be able to meet the Environmental and Physical Demands of the job as found in the attached Statement.
  • Exercise authority to investigate, resolve, and properly report complaints or incidents to ensure compliance with applicable laws, rules, regulations and policies.
  • Mediate staff disputes and clarify staff misunderstandings to ensure effective teamwork and positive tenant oriented outcomes.
  • Be dependable in performing tasks, duties, attendance and an example for others.
